The explanation was received from the concerned Secretary who has typed the order dated 16-11-2017 which is kept at flag "Y".

In the show cause, it has been stated by the concerned Secretary that he was in shock because he received shocked message on mobile about sudden death of his younger sister aged about 36 years. Therefore, such mistake has occurrerd. The explanation submitted by the concerned Secretary is accepted. However, the concerned Secretary is warned to not repeat such type of mistake in future. However, warning be entered in the ACR of concerned Secretary.
