Mr. Walve states that annexed as Exhibit "K" to the petition is a print out of Form - 4 that has been filed by petitioner. Mr. Walve, on instructions, states that within four weeks from today respondent will consider the Form - 4 filed and issue Form - 5 under the VSV Scheme unless respondent finds some other objection/violation. Statement is accepted as undertaking to this Court.
