Learned counsel appearing for respondents, on instructions, states that if the original corrected form a copy of which is annexed at Exhibit "A" to the petition, is submitted by the petitioner to the Medical Officer of Health, necessary action in accordance with law shall be taken on the basis of said form. She states that original form was collected by one Anil Shamrao Rawate on 6th December 2016, for carrying out necessary correction, but said form has not been returned.

Hence, we direct the respondents that as and when the petitioner submits the original corrected form a copy of which is annexed at "A" to the petition, the concerned Officer of the Mumbai Municipal Corporation shall take appropriate decision on the letter dated 24th November 2016, submitted by Agrawal Nursing Home to the Medical Health Officer, H(W) Ward. Appropriate decision shall be taken within a period of one month from the date when such corrected form is submitted by the petitioner. 5.

The petition is disposed in above terms.
