Reply affidavit of the insurance company is placed on record. 2.

In terms of the reply affidavit, the writ petition can be disposed of. Therefore, we have taken up the matter for disposal. 3.

Petitioner has approached this Court seeking benefits of Pay Revision Scheme, 2010, so also, payment of Provident Fund contribution made by him. After going through the record, learned counsel for petitioner fairly admits that the benefits of Pay Revision Scheme, 2010, are not applicable to the petitioner. So far as payment of Provident Fund amount, it is stated that since proper application in the format prescribed was not submitted, the same could not be processed. Respondents agree that once proper

{2} wp7375.16.odt application in the prescribed format is submitted, the same would be processed and forwarded to the Commissioner of Provident Fund for the needful.

In the light of above submissions, petition is disposed of directing the petitioner to submit proper application in the format prescribed, within two weeks from today and, the same shall be processed and forwarded to the concerned authority for doing the needful within eight weeks from the date of receipt of application from petitioner.
