The petitioners shall file typed copy of medical report and shall also obtain certificate from Dr. H. J. Sabley as regards status of health subsequent to 8-8-2021 and the prescription and/or medicines and/or treatment given to patient with details of recovery of ailment. The certificate should also disclose the reasons why should patient avoid public contact.

The petitioners shall submit statement on affidavit as to whether the husband of patient attended duties on and after 8-8-2021.
