I am the applicant herein. I am filing my examination in chief in the form of the proof affidavit. In continuation of the same, now I am deposing and marking the following documents.

Ex.A1 (series) are the photocopies of the medical records of Mrs.V.Vardhini, the respondent herein (compared with the original).

Ex.A2 is the photocopy of the lab report dated 18.11.2023 of Mrs.V.Vardhini, the respondent herein (compared with the original). Ex.A3 is the photocopy of the doctor's prescription dated 20.11.2023 of the respondent.

Ex.A4 is the photocopy of the doctor's prescription dated 22.02.2024 of the respondent.

Ex.A5 (series) are the doctor's prescription and pathology report of the respondent.
