O R D E R

The petitioner seeks a writ of mandamus directing the respondents to disburse the Employees provident Fund amount and Contributory Provident Fund amount due to her husband, who died on 13.08.2009.

2. The learned Additional Government Pleader, on instructions from the 1st respondent submitted that a specimen Form -14 has already been sent to the petitioner and on receipt of the same, appropriate orders will be passed.

3. The learned counsel for the petitioner submitted that the petitioner has not received Form 14 from the 2nd respondent. According to the learned counsel, the petitioner is prepared to sign Form -14 for onward transmission to the 1st respondent.

4. The petitioner is directed to appear before the District Forest Officer, Interfaced Forest Division, Krishnagiri on any working day next week for the purpose of signing Form 14. The 2nd respondent should see that signature is obtained in Form 14 from the petitioner and forward it to the 1st respondent for passing appropriate orders. The 1st respondent is directed to pass appropriate orders and release EPF and CPS amount along with interest in accordance with law. Such exercise shall be completed within a period of four weeks from the date of receipt of Form 14 from the 2nd respondent.

The Writ Petition is disposed of with the above direction. No costs.
