The relief sought for in this writ petition is to direct the respondents to reimburse the medical claim of the petitioner based on his representation dated 26.06.2021.

2. Medical reimbursement claims are to be considered in accordance with the Health Insurance Scheme and by scrutinizing the application and medical records of the applicant.

3. The petitioner states that he submitted an application on 26.06.2021 for medical reimbursement. However, it is not made clear whether the application is in the prescribed format along with necessary documents or not.

4. This being the factum, the petitioner is at liberty to submit a copy of the application along with required documents to the first respondent, along with a copy of this order, within a https://hcservices.ecourts.gov.in/hcservices/ ____________

W.P.(MD) No.14998 of 2021 period of two weeks from the date of receipt of a copy of this order. On receipt of the same, the first respondent is directed consider the same in accordance with the terms and conditions of the Health Fund Scheme and pass orders on merits and in accordance with law, within a period of eight weeks thereafter.

5. Accordingly, the writ petition is disposed of. No costs.
