4. The facts as set out in the affidavit filed in support of the writ petition reads as follows:- The petitioner would submit that her daughter namely D.Agalya was born on 21.01.1988 at Government Rajaji Hospital, Madurai and birth certificate was issued by the Registrar of Births and Deaths, Madurai Corporation. Due to lack of awareness, her daughter's name had not been incorporated in the birth certificate. However, in Aadhar Card, Ration Card, Voter ID and Educational Records, her daughter's name has been recorded as D.Agalya.
5. She would submit that since birth certificate has become mandatory for official and employment purposes, her daughter's name has to be incorporated in the birth certificate. In this connection, she had sent a representation dated 15.09.2025 along with requisite fee and the same has not been considered. Thereafter, again she had sent a representation dated 03.10.2025. However, no action has been taken to date. Aggrieved by the same, the petitioner has come before this Court. 3/6
