The matter has appeared in the list at the instance of the writ petitioner under the heading 'To Be Mentioned' for correction.

The third paragraph of the first page of the order dated 22nd September, 2021 should read as follows:- "The petitioner is an Associate Professor in Chandidas Mahavidyalaya. The petitioner initially used the middle name 'Kumar' and surname 'Pramanick'. He later omitted his middle name and changed his surname from 'Pramanick' to 'Roy' and published the same in the Kolkata Gazette Extraordinary on 26th October, 2017. Relying upon the aforesaid gazette notification the Adhaar Card, PAN Card and banking documents have incorporated his present surname 'Roy'."

The operative portion of the order should read as follows:- "the writ petition is disposed of by directing the respondent no. 2 being the Director of Public

Instruction to take necessary steps for effecting changes in the name of the petitioner by deleting his middle name 'Kumar' and incorporating his surname 'Roy' in his service book, at the earliest, but positively within a period of eight weeks from the date of communication of a copy of this order."

The remaining part of the said order will remains unaltered.

The department is directed to carry out the necessary correction at once.

Urgent photostat certified copy of this order be supplied to the parties, if applied for, as early as possible.
